Application Forms

Below you will find links to all the forms needed to submit an application to Paideia School.  In order to view, fill in or print these forms you will need a free program called Adobe Acrobat Reader.  You may either print the forms and fill out the information by hand or type the information on the computer and print.  The Admissions Office has no preference for either handwritten or typed forms, only that all information be legible.  Completed forms must be mailed or delivered to Paideia.  They cannot be submitted online.

  • To complete the forms on the computer, move the mouse to a line. Click when the hand changes to a straight line cursor and begin typing. The hand will change to a pointing finger over a checkbox.  Click to select that response.
  • To move from line to line, press Tab or click on the new line.
  • For longer responses, the number of lines permitted is noted in blue. Do not scroll down in the text box past the stated number of lines as they will not be visible when the form is printed. You are welcome to attach a separate sheet for longer responses than available in  the space on the form.
  • You may or may not see the printed lines on the forms when you are typing a response (different computers will display differently). If the lines are not visible while you are typing, they will reappear when you tab or click out of that space. It's ok if a line of your response prints on top of a line on the form.
  • The instructions in blue will not be visible when the forms are printed.
